Many have this common misconception that it’s either Tummy tuck (Abdominoplasty) or Liposuction. However it’s all about age, weight gain, pregnancy or some combination of those factors. For example, If you decided to go for liposuction, and you have a huge amount of fat, the skin will not retract and will hang. In contrast, abdominoplasty removes not just the fat but also the extra skin and tightens up the muscles.

Major Difference between Liposuction and Tummy Tuck

Tumescent Liposuction

Tummy Tuck (abdominoplasty)

tummy tuck or abdominoplasty

Mechanism of Action

It is merely a technique to remove fat. The usual application is in body-contouring as an elective cosmetic procedure. However, the technology of liiposuctioning can be applied to other situations such as breast reduction, contouring of flaps including those for breast reconstruction, and debulking fatty tumors. Whether or not applications in these possibly medically-indicated situ

It is reserved solely for the midsection and is a fairly major surgical procedure. During a tummy tuck, an incision is made in the lower abdominal region, weakened muscles and fascia of the abdominal wall are restored and tightened, loose skin and tissue is removed, and the stomach-area is flattened and tightened.

Major Benefits

Target trouble spots in one who is obese. including areas around the face, legs, chest, flanks, back, arms and buttocks.

You get rid of the extra skin and fat, and you tighten up muscles that have stretched out with either pregnancy or weight gain.

Downtime – Adverse effects

– Don’t have a long scar,

– You to spend a night in the hospital,

– You are back to work quicker. Usually you finish operation and can have normal day afterwards on the same day (outpatient)

– Leaves a long scar down low

-Recovery is somewhat harder because it is a bigger operation,

– You also have to spend a night in the hospital and take at least two weeks off from work and duties around the home to allow your body to recover.

Indicated for

For patients who require less skin and muscle tightening and would like to just smooth and improve contour in a few areas, liposuction may be a better option – For patients who have recently lost a large amount of weight in a short period of time are often left with loose, sagging abdominal skin which can easily be removed with a tummy tuck.

– Tightening both skin and muscle is very important, especially for women who have had multiple pregnancies.

– older patients 50+ (skin loses elasticity with age)

Sedation Used

Local Anesthesia

Tumescent local anesthesia, also referred to as TLA or Klein’s Solution, is a carefully balanced blend of lidocaine, epinephrine and sodium bicarbonate diluted in saline and administered directly into the subcutaneous fat layers to produce total anesthesia and vasoconstriction lasting effectively up to 12 hours.

General Anesthesia

Time Frame

The average patient will be in the surgical facility for 3 to 7 hours. However, the actual surgery may only require 1 to 3 hours. surgical time is usually from about 2.5-4 hours, depending on exactly what has to be done (a “tummy tuck” after significant weight loss cannot reasonably be compared to a small tummy tuck after one pregnancy in which very little weight was gained, etc).  1-3 hours can be added for liposuction and additional tissue removal including the back, etc.  So the total is typically about 2.5-5 hours – although surgeons operate at different speeds

If pain is controlled during surgery it does not escalate after.
Pain after liposuction varies with the areas being done but most people describe the sensation as soreness like after a vigorous work out.
You will feel tight, swollen, and not be able to stand fully upright. Your tummy will hurt but the pain can be controlled with pain pills.

Post –Op Complications

There is little to no complications with tumescent liposuction. The complications arises if more than 5 liters are sucked in one session. However, all professionals never do this and their limit is always 5 liters. Emotional swings do occur postoperatively.

Recovery ranges from 2-4 weeks depending on the amount of surgery that is performed. However usually you get back to normal after 2 weeks.

We advise patients to refrain from any heavy lifting or exercise for at least 3 weeks from the time of surgery.

Combination of both surgery includes

1. Removal of skin and fat

2. Liposuction of the abdomen & Hips

3. Repair of separated muscles (diastasis recti)

Simple steps of Tummy Tuck

A tummy tuck which is also referred to as an abdominoplasty is a procedure to enhance the appearance of the abdomen.  It consists of 3 basic steps:

  1. Skin removal and tightening
  2. Subcuataneous fat removal
  3. Muscle tightening

Tummy Tuck Post-op in details

After operation you will be able to get up to go to the bathroom with minimal assistance.  You will get a pain pump to help control the pain for the first several days.  Narcotic pain medications for 2-3 weeks, is a must. A follow up is pretty much weekly for the first month.

You will be able to stand straight after 1-2 weeks, begin driving 2 weeks, begin exercises after 4 weeks, be back to most all activities 2 months, and 12-18 months for numbness to resolve and scars fade. Those general guidelines do not tell you exactly how you’ll recover.

You may notice, for several months, that certain things you do cause twinges, tugs, etc. You may notice new areas becoming tingly, sensitive or sore as nerves heal and recover. Most of this is a minor annoyance and does not stop you from doing simple activities. It all tends to get better over time. Truth be told recovery is rough after Tummy Tuck operation.

Tummy Tuck Before and After Photos

in the after photo you can see scars due to major amount of skin. These results varies. But pictures give a clear understanding of realistic results.  The after pictures are 12 weeks after operation.

When to initially decide to do both (liposuction + Tummy Tuck)?

You simply have to answer these 3 questions

  1. How thick is your “fat layer” ? is it too thick for your liking ? Can you grab it in your hand and feel excess fat ? If this is true, then you need to either lose weight by dieting or you may be a candidate  for liposuction to reduce the thickness of this layer
  2. Do you have excess skin that is a significant issue for you ? Women are individuals and tolerate certain amount of lax skin differently, but if it is intolerable to you then skin tightening is mandatory. The gold standard for tightening skin is through an abdominoplasty in which the entire abdominal skin is elevated along with the fatty layer and the excess skin is removed.
  3. Is there abdominal wall / muscle laxity. This assessment often requires your surgeon’s evaluation to be sure. However if you feel below the fat layer and can tell that not only is your abdomen bulging due to fat, but also due to a protuberant abdominal wall, then the muscle layer must be tightened.

Main downside of Tummy Tuck

This is a wonderfull operation that will make the tummy flat FOREVER, WITHOUT SIT-UPS!!  But, you have to be willing to trade the appearance for the scar, which is long, but well placed, and hidden in all clothing and bathing suits.
